Poor line quality. Suggestions?

I've had Uverse at this location since early October 2014. It's been relatively stable since then with few hiccups. Recently, we've had crashing and freezing and loss of connectivity watching TV. A tech had to come out due to inability of connectivity. The tech on the chat line had sent a new box that had the same issue so a tech was dispatched. He spent hours trying to fix things but eventually came to the conclusion that the speed tier had to be reduced. After that was changed connectivity was reestablished but unfortunately things have been glitchy. The numbers are as follows from the NVG589 attached
